1986: 73 seconds into its launch, the Space Shuttle Challenger broke apart killing all seven crew on board. The much publicised mission got the go for launch on a frosty morning - against the advice of engineers.
An investigation had found that one of the rubber O-rings that sealed the joints in the solid rocket boosters had failed, leading to breach of the joint by the hot, high pressure fuel. The O-rings failed due to the cold conditions, making them too brittle for the job they had been designed for.
